All GBL Teams must be certified. This will require assembling a certification book that must be completed and signed by October 23, 2006. Once completed, the books must be signed by either Kyle Anderson or Bob Evans authorizing the teams for competition in the Gwinnett Basketball League as representatives of the Grayson Cluster.

Each Certification Book should contain the following:

1. Player Registration form, copy of birth certificate, and copy of current report card ( one for each player .)  Both the player and one guardian must sign their form.  Players attending private school must additionally provide a utility bill demonstrating proof of residency within the Grayson Cluster school district.

2. Coach Registration form ( one for each team .)  Complete a single form for the team and have all coaches sign the same form.

3. Team Registration form ( one for team .)  Complete the form clearly and legibly with all Coaches signing the same copy.

4. Assemble all documentation into a book. A 1" 3-ring binder with all papers in the following order:

Current Report Card

NOTE:  These books will be turned into Gwinnett Basketball League and held until the completion of the end of season tournament. When requesting documents from parents, be sure to tell them to provide copies that do not need to be returned.
